Every cadet is required to complete the forms below.

Cadet Information Form

• Cadet demographic information required by the Navy                                                         ​Cadet and parent/guardian contact information required by the program.​
  

NJROTC STANDARD Release form

• All entries on the Navy Standard Release form must have information entered.             Enter "none" or N/A if not applicable.
  

STUDENT / PARENT CONSENT FORM

• Consent of parent(s)/guardian(s) and student to participate in Navy JROTC                     ​Cadet and parent/guardian signatures are required.
  

PERMISSION TO TREAT INJURY

• Authorization to treat student injuries.

PLCS SIGNATURE FORM

• PLCS policies related to student health, hazing, team rules, and sportsmanship.          Cadet and parent/guardian signatures are required.
  

Transportation policy

• PLCS District student conduct expectations when using district transportation.            ​Cadet and parent/guardian signatures are required.
